Clinical Service Coordinator

River Edge is looking for an experienced licensed or license‑eligible Clinical Service Coordinator with a Master’s degree in Social Work, Psychology, Counseling or Behavioral Health related fields to work at our Adult Outpatient Center. The role involves completing behavioral health assessments on primarily adult populations with mental health or dual diagnoses. We seek a candidate who wants to build experience with a wide range of behavioral health conditions in a professional atmosphere at an organization striving to be a leader in the nation.

Summary of job duties:
  • Performs and tracks needs for biopsychosocial and behavioral health assessments, treatment planning/monitoring and discharge planning, in person or via telehealth for clients participating in mental health group training and peer support groups.
  • Completes assessments for appropriateness for the group, including referrals from NGRI/IST, parole/probation, regional referrals, hospital discharges, and internal referrals.
  • Assesses and refers to internal programs such as case management, substance‑abuse outpatient programs, supported employment services, early intervention programs for individuals experiencing their first episode of psychosis, intensive case management, ACT services, residential programs, or crisis services.
  • Collaborates with group leaders to develop treatment plans and appropriate interventions for individuals served in the group.
  • Discharges and transitions clients to other programs accordingly.
  • Updates client records including contact information, ROIs, etc.
  • Completes documentation or calls to report to a personal care home, court, probation/parole, treatment teams, NGRI/IST and other partners.
  • Reviews conditional release plans and makes appropriate modifications.
  • Completes reports and attends monthly NGRI/IST meetings.
  • Attends monthly supported employment meetings and gives updates on individuals served in group and supported employment.
  • Performs commitment determinations and/or executes orders of involuntary commitments.
  • Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement/education, etc.
  • Maintains a safe, secure, and healthy work environment by following and enforcing set standards and procedures and complying with legal regulations.
  • Adheres to professional standards, policies and procedures, federal, state, and local requirements, ASO, DBHDD, and Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
  • Updates job knowledge by maintaining licensure, participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
Minimum Qualifications:
  • Master’s degree in a behavioral health field and eligibility for a Georgia license as LCSW/LMSW, LPC/APC, or LMFT/LAMFT. Must actively pursue licensure upon hire.
