CenterLight Healthcare PACE in New York seeks a Clinical Service Director to lead interdisciplinary teams, align clinical and operational performance, and drive participant-centered care plans for aging adults in the community.

This full-time role offers a Monday–Friday schedule (8:30 AM–5:30 PM) with a hybrid work model, a remote day after training, and a comprehensive benefits package including PTO, holidays, retirement plan, and professional development opportunities.

Qualifications

• Active and unrestricted RN license in New York.
• BSN degree required.
• Minimum 3 years acute care nursing experience.
• Experience with elderly populations in PACE, MLTC, CHHA, or similar settings.
• Demonstrated leadership experience.

Responsibilities

• Lead IDT operations and participant outcomes.
• Direct and support clinical and operational managers across multiple service lines.
• Develop and evaluate participant-centered care plans.
• Monitor operational, quality, and clinical performance metrics.
• Collaborate with organizational leaders to enhance care delivery processes.
• Oversee incident investigations and participant concerns.
• Ensure clinical documentation is accurate and compliant.
