Job summary

CenterLight Healthcare PACE is seeking a licensed RN to join a NYSNA union-backed program in the New York area. The role involves delivering skilled nursing care to participants in their homes, performing UAS-NY assessments, and collaborating with an interdisciplinary team to tailor care plans.

You will coordinate with physicians and facilities, administer medications, educate participants and caregivers, and document care comprehensively across the continuum.

Qualifications

• Current New York State RN license.
• Graduate of an accredited nursing program.
• Minimum one year RN experience in Home Health, Acute Care, Medical-Surgical and/or Critical Care.
• Valid driver's license and reliable vehicle.
• Computer proficiency.
• Strong communication and organizational skills.

Responsibilities

• Provide skilled nursing care in participants' homes and across the continuum of care.
• Complete comprehensive nursing assessments and UAS-NY evaluations.
• Develop and update individualized care plans with the IDT.
• Coordinate care with physicians, hospitals, specialists, nursing facilities and community providers.
• Administer medications and skilled nursing treatments.
• Educate participants and caregivers on disease management, medications and safety.
• Identify changes in condition and intervene early to prevent avoidable hospitalizations.
• Complete timely, accurate clinical documentation.

37.5 Hours/Week | Monday-Friday 8:30 AM-5:00 PM Union Position (NYSNA) Salary Range: $114,497.00 - $131,827.72* * Position's wages are contingent upon terms and conditions outlined in the collective bargaining agreement. Eligibility for additional compensation based on verified nursing experience and qualifying nursing education in accordance with the NYSNA Collective Bargaining Agreement. Bring exceptional nursing care directly to the community. At CenterLight Healthcare PACE, you'll build lasting relationships with older adults while helping them remain safe, healthy, and independent in their own homes. Unlike traditional home care, you'll be part of an Interdisciplinary Team (IDT) where your clinical expertise directly shapes each participant's individualized care plan.

What You'll Do

What We're Looking For

Required Qualifications

• Current New York State Registered Nurse (RN) license
• Graduate of an accredited nursing program
• Minimum one year of RN experience in Home Health, Acute Care, Medical-Surgical and/or Critical Care
• Valid driver's license and reliable vehicle
• Computer proficiency
• Strong communication and organizational skills

Preferred Qualifications

• BSN
• Experience with frail or elderly populations
• UAS-NY assessment experience
• PACE, Managed Care, Home Care or Case Management experience

Salary Range (Min-Max): $114,497.00 - $131,827.72 Note: Position's wages are contingent upon terms and conditions outlined in the collective bargaining agreement. CenterLight Healthcare PACE, a Program of All-Inclusive Care for the Elderly, serves adults 55+ throughout New York City, Westchester, Nassau, and Western Suffolk Counties. Designed for those with Medicare and/or Medicaid, the program empowers older adults to continue to live independently at home and in their communities. CenterLight’s PACE program is one of the largest nonprofit PACE programs in the country and operates 11 PACE centers and one Alternative Care Setting (ACS) where participants can receive on-site socialization, social and personal care services, therapeutic recreation, meals, and transportation.
