Job summary

Your Health in West Columbia is seeking a Clinical Scheduler to support our Care Manager role and assist with all scheduling needs for West Columbia patients. The role serves as the central scheduling and operational coordinator for a single interdisciplinary care team, ensuring patients receive the appropriate number and type of in-person encounters under our proactive, value-based care model.

This is a full-time, salary-based 8-hour position (8:00 AM–5:00 PM) Monday through Friday, located in

Qualifications

High school diploma or equivalent required; an Associate's or Bachelor's Degree in Healthcare is preferred.
Preferred CMA, LPN or RN
Previous experience in a medical or healthcare setting, preferably in scheduling or administrative role.
Familiar with medical terminology, procedures, and insurance verification.
Strong organizational and time management skills; multitask and prioritize responsibilities.
Excellent communication and interpersonal skills.
Ability to work under pressure.
Strong problem-solving abilities.

Responsibilities

Register, verify and timely schedule patients according to the Company’s care model.
Track and follow up with new patients until scheduled.
Schedule appointments according to care model including follow-ups with healthcare providers based on availability, medical urgency, and patient preferences.
Ensure all disciplines with schedules are fully optimized.
Ensure patients are being followed and scheduled appropriately when discharged from an in-patient facility.
Ensure TCM appointments are scheduled promptly and within the required window.
Ensure strong documentation is implemented.
A multitude of software systems (including tracking systems) will be utilized to ensure patient follow-up and better patient outcomes.
Interact with patients via email, patient portal, telephone, AthenaText, in-person, etc. to gather necessary information, schedule and confirm appointments, and provide pre-appointment instructions.
