Job summary

Your Health Organization in South Carolina is seeking a Care Manager/Clinical Scheduler to coordinate all in-person visits for the Okatie Clinic team and ensure the right number and type of encounters under our value-based care model.

This is a full-time, 8-hour-per-day role from 8:00 AM to 5:00 PM, Monday through Friday, focusing on scheduling, patient communication, and cross-functional collaboration with providers and facilitators.

Qualifications

• High school diploma or equivalent required; an Associate's or Bachelor's Degree in Healthcare preferred.
• Preferred CMA, LPN or RN.
• Experience in medical or healthcare scheduling or administrative role.
• Familiar with medical terminology, procedures, and insurance verification.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track and follow up with new patients until scheduled.
• Schedule appointments based on care model, availability and urgency.
• Optimize schedules across disciplines and provider encounters.
• Follow patients after discharge and ensure timely follow-ups.
