• Counsels patient/caregiver in meeting nursing and related needs.
• Assigns and coordinates services provided.
• Assists physician in developing a plan of treatment/care.
• Prepares clinical progress notes ensuring changes in patient condition are reported.
• Monitors clinical records for complete and accurate documentation.

• Pay per Visit
  • Sup/Post Hospital: $60
  • Reassessment: $75
  • Initial Admission: $90 weekday/ $125 weekend
• Visiting Client Territory: Liverpool, and surrounding areas.
• Schedule: Visits planned with the required presence of the patient's aide.
