The Care Manager collaborates with providers, interdisciplinary teams, and community partners to improve health outcomes and reduce barriers to care. Using clinical judgment, crisis intervention skills, and evidence-based guidelines, they support members through complex health challenges and promote engagement in their treatment plans. The position also focuses on quality improvement, regulatory compliance, and cost-effective care management. Success in this role requires strong clinical expertise, communication skills, and the ability to work independently while managing multiple cases.

Skills and Requirements

Licensed Clinical Social Worker (LCSW/LCPC)3+ years behavioral Health Clinician experience, orBehavioral Health experience plus 2 years working with complex populations
