Vi at Palo Alto is located at 620 Sand Hill Road, Palo Alto CA 94304.

Responsibilities:

Collaborates with the DON, CRM (where applicable), and various disciplines to assist in the completion of the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and all associated processes. Ensures completion of assessments within the mandated timeframe, manages resident assessment processes, tracks Medicare/Managed Care/Medicaid case documents for optimal reimbursement, and assists with resident care planning under the direction of the DON or CRM. Performs other duties as assigned.

THIS IS A PART TIME POSITION.

Qualifications:

• Graduation from a School for Registered Nursing.
• Minimum 1 year of experience in an MDS/Clinical Reimbursement Nurse role preferred.
• Current State Licensure as a Registered Nurse, CPR, and AED certifications required. First Aid certification required or willingness to obtain. Food handler certification if required.
