Job summary

Immanuel Pathways PACE seeks a Clinical Referral & Scheduling Specialist to coordinate referrals and scheduling across its three PACE centers. The role requires LPN or MA certification, with Epic/EHR experience and strong coordination skills to ensure timely access to care.

You will serve as a centralized resource for Clinic Schedulers, ensuring compliance, accurate documentation, and efficient referral workflows while supporting patient-centered care and organizational goals.

Qualifications

• Completion of an accredited practical nursing or medical assisting program is required.
• Current LPN licensure or certification/registration as a medical assistant is required.
• Two years in clinical support, scheduling, or care coordination is preferred.
• Experience coordinating complex referrals and scheduling using EHRs is preferred.

Responsibilities

• Coordinate and schedule referrals and appointments across Immanuel Pathways PACE centers.
• Schedule and document referrals within Epic with complete and timely records.
• Act as centralized scheduling resource and guide clinic schedulers on terminology and requirements.
• Identify and resolve missing or unclear referral information to prevent delays.
• Support quality improvement and streamline referral workflows.
