Job summary

Autumn Hospice is hiring a Clinical Records Coordinator to manage patient records from admission to discharge, ensure accuracy, and maintain HIPAA compliance. You will coordinate with physicians, facilities, and IDT, while handling both electronic and paper records.

The role emphasizes meticulous documentation, timely certifications, and collaboration with a multidisciplinary team in a compassionate hospice setting.

Qualifications

  • Experience with medical terminology.
  • Experience working with both electronic and paper medical records.
  • Minimum of one year of related healthcare or medical records experience.
  • Strong organizational, communication, and time management skills.
  • Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

  • Initiate and maintain patient clinical records from admission through discharge.
  • Prepare, organize, track, and file all required medical record documentation.
  • Ensure patient charts are complete, accurate, and compliant with policies and regulations.
  • Maintain electronic and paper medical records with HIPAA confidentiality.
  • Track physician orders, certifications, recertifications, plans of care, and other documents.
  • Coordinate DME orders and communicate equipment issues with leadership.
  • Serve as the primary contact for medical record activities between office, physicians, facilities, and IDT.
  • Retrieve, release, and copy records for authorized personnel.

Clinical Records Coordinator Responsibilities

The Clinical Records Coordinator is responsible for, but not limited to:

  • Initiate and maintain patient clinical records from admission through discharge.
  • Prepare, organize, track, and file all required medical record documentation.
  • Ensure patient charts are complete, accurate, and maintained in accordance with company policies and state and federal regulations.
  • Maintain both electronic and paper medical records while ensuring confidentiality and HIPAA compliance.
  • Track physician orders, certifications, recertifications, plans of care, and other required clinical documents.
  • Coordinate Durable Medical Equipment (DME) orders and communicate equipment issues with clinical leadership.
  • Serve as the primary point of contact for medical record activities between the office, physicians, facilities, and the interdisciplinary team.
  • Follow up with physicians to obtain signed certifications and other required documentation in a timely manner.
  • Retrieve, release, and copy medical records for authorized personnel in accordance with company policies and applicable regulations.
  • Prepare clinical documentation and reports for Interdisciplinary Team (IDT) meetings, including census reports, admissions, discharges, transfers, medication profiles, certifications, and recertifications.
  • Monitor and report patient census information.
  • Participate in daily clinical communication to ensure patient status changes are accurately documented and communicated.
  • Perform routine clinical record audits and promptly identify and resolve missing or incomplete documentation.
  • Support staff in maintaining timely and accurate clinical documentation.
