We serve all people, regardless of income or insurance status. Affinity Health Center seeks a vibrant and motivated Clinical Quality Coordinator to champion our patient care quality program. The coordinator will use strong relationship building skills to collaborate with primary care clinical teams and leaders on strategies to support closure of care gaps and positive patient health outcomes as measured by standardized indicators.

The highly organized Coordinator has a detailed understanding of health care quality goals in closing care gaps, monitors performance and tracks agency data performance in comparison with regional and national standards. The Coordinator uses Continuous Quality Improvement methodology to develop and implement processes and facilitate initiatives across the organization to ensure that Affinity Health Center has a pathway to help close patient care gaps.
$19.00 – $24.00 Per Hour
