Job summary

Metropolitan Jewish Health System, Inc. seeks a Clinical Program Liaison to evaluate and plan care for patients referred to Hospice Care services, working on-site at a major medical center.

You will join the business development and marketing team and build relationships with physicians, social workers, case managers and discharge planners. Candidates should have a Bachelor’s degree in Social Work and hold LCSW/MSW/LMSW, plus at least two years’ experience in Medical-Surgical or Community

Qualifications

• Bachelor’s degree in Social Work or related field; LCSW/MSW/LMSW preferred.
• Minimum of two years’ experience in Medical-Surgical or Community Health; hospice experience preferred.
• Proficient with MS Word and Excel; able to document and coordinate plans.
• Ability to work flexible hours to meet patient care needs.
• Strong teaching, communication, and listening skills; culturally sensitive.
• Ability to collaborate within an interdisciplinary team.

Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.

Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

• Bachelor’s degree from an accredited School of Social Work; LCSW, MSW or LMSW


• A minimum of two years’ experience in either Medical-Surgical or Community Health. Hospice experience preferred.
