Job summary

Metropolitan Jewish Health System seeks a Clinical Program Liaison to evaluate and establish care plans for Hospice referrals and to work within the hospital setting. You will be a key member of the business development and marketing team, fostering relationships with physicians, social workers, case managers and discharge planners.

• Bachelor’s degree from an accredited School of Social Work; LCSW, MSW or LMSW.
• Two years’ experience in Medical-Surgical or Community Health; Hospice experience preferred.
• Microsoft Word and Excel proficiency.
• Flexible hours to meet role needs and coordinate interdisciplinary resources.
• Teaching, communication and listening skills are essential.
• Ability to work non-judgmentally with patients and families from diverse backgrounds.
• High degree of accountability and strong organizational skills.
• Ability to speak in public and work within an integrated health care team.

Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

Qualifications

• Bachelor’s degree from an accredited School of Social Work; LCSW, MSW or LMSW
• A minimum of two years’ experience in either Medical-Surgical or Community Health. Hospice experience preferred.
• Microsoft Word and Excel
• Ability to work flexible hours to meet the needs of the position
