MJHS seeks a Clinical Program Liaison to lead hospice care planning for patients referred to our services. The role is on-site at a major medical center in New York, working with physicians, social workers, and discharge planners to develop comprehensive care plans and ensure seamless service delivery.
Ideal candidates hold a Bachelor’s in Social Work with licensure (LCSW/MSW/LMSW) and have at least two years of Medical-Surgical or Community Health experience, including hospice exposure.

Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.
