Job summary

MJHS seeks a Clinical Program Liaison to lead hospice care planning for patients referred to our services. The role is on-site at a major medical center in New York, working with physicians, social workers, and discharge planners to develop comprehensive care plans and ensure seamless service delivery.

Ideal candidates hold a Bachelor’s in Social Work with licensure (LCSW/MSW/LMSW) and have at least two years of Medical-Surgical or Community Health experience, including hospice exposure.

Qualifications

• Bachelor’s degree from an accredited School of Social Work is required.
• LCSW, MSW, or LMSW licensure or eligibility preferred.
• Two years’ experience in Medical-Surgical or Community Health; Hospice experience preferred.
• Proficiency in Word and Excel; strong communication and organizational skills.

Responsibilities

• Evaluate and establish a plan of care for patients referred for Hospice Care services.
• Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center.
• Coordinate resources and personnel in an interdisciplinary team environment; participate in business development and marketing activities.

Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.
Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

• Bachelor’s degree from an accredited School of Social Work; LCSW, MSW or LMSW
• A minimum of two years’ experience in either Medical-Surgical or Community Health. Hospice experience preferred.
