Job summary

Carle Health is recruiting a licensed Physical Therapist to evaluate and treat patients of all ages in a hospital/rehabilitation setting. You will design care plans, monitor progress, and collaborate on discharge planning with the medical team.

This role emphasizes patient and family education, ongoing documentation, and supervision of PTA/rehab staff to ensure high-quality care and regulatory compliance, with IL licensure and BLS requirements.

Qualifications

• Licensed Physical Therapist (PT) in IL
• Basic Life Support (BLS) within 30 days
• Attend CE for acute stroke care

Responsibilities

• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan
• Educates patients, family, and caregivers using effective teaching methods
• Evaluates clinical findings to determine PT diagnoses and outcomes of care
• Performs examination using reliable methods relevant to chief complaint and history
• Documents in accordance with CTSCP67 and regulatory standards
• Screens patients to determine therapy needs and uses support personnel per guidelines
• Supervises PTA and rehabilitation technicians
• Communicates patient performance and discharge recommendations to the medical team

Education

Bachelor's Degree in Physical Therapy
