Qualifications

Responsibilities

• Designs an appropriate plan of care integrating goals, treatment, outcomes and discharge plan.
• Educates patients, family and caregivers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using valid methods related to chief complaint, screening, and history.
• Documents care in accordance with CTSCP67 and organizational standards.
• Supervises PTA and rehab technicians per regulatory guidelines.
• Communicates patient status, discharge recommendations and equipment needs to the medical team.
