Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care to outcomes and evidence. You will educate patients and families, perform PT examinations, and document in line with policy.

Qualifications

Education: Bachelor's Degree in Physical Therapy.
Licenses: Licensed Physical Therapist (PT) - IL and BLS (AHA).
Continue education to stay current in stroke care.

Responsibilities

Designs an appropriate plan of care integrating goals, treatment, outcomes and discharge planning.
Educates patients, family, caregivers, students and other health care providers.
Evaluates clinical findings to determine PT diagnoses and outcomes.
Performs examinations and selects appropriate assessment methods.
Produces documentation per CTSCP67 policy.
Screens patients to determine effectiveness of therapy and needs for services.
Supervises PTA and rehab technicians as required.
Communicates patient status, discharge recommendations, and equipment needs to medical team.
