Responsibilities

• Evaluates PT needs and develops goals for individuals with complex conditions.
• Performs hands-on therapy, procures adaptive equipment as needed, oversees equipment repair.
• Coordinates care with interdisciplinary team to optimize resource use and patient safety.
• Provides supervision to Physical Therapy Aides and Assistants.
• Attends meetings, rounds, and coordinates post-care planning with clinicians.
