Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license/registration in North Carolina.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form according to guidelines.
• Provide a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatments using modalities and adjust care as indicated while following precautions.
• Document daily participation and progress on weekly progress notes.
• Update treatment plans in a timely manner and prepare discharge summaries as needed.
• Prepare care plans and information for the MDS as required by the facility.
• Collaborate with the interdisciplinary team and participate in facility meetings.
