Job summary

Warsaw Rehabilitation And Healthcare Center in Warsaw, NC is hiring a full-time Physical Therapist (PT). You will perform comprehensive evaluations, develop treatment plans, and provide skilled physical therapy to residents in collaboration with the interdisciplinary team.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.

Responsibilities

• Provide comprehensive physical therapy evaluation based on MD orders.
• Document findings on standardized evaluation forms and adhere to guidelines.
• Create and implement comprehensive treatment plans with long/short-term goals, frequency, and modalities.
• Ensure MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver treatments using appropriate modalities and adjust plans as needed.
• Document daily participation and progress in weekly notes.
• Update treatment plans and discharge summaries as required.
• Document caregiver or discharge in-services as needed.
• Complete all required documentation for the facility.
• Prepare care plans and information for the MDS as needed.
• Attend facility and rehab meetings.
• Participate in committee meetings as indicated.
• Act as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and performing screens.
• Complete billing accurately for PT services.
• Cover other therapists' caseload during absences.
• Supervise junior staff or students as needed.
• Maintain professional relationships with staff, patients, families.
• Provide in-service education on patient care to rehab staff or community.
• Comply with facility policies and patient confidentiality.
