Job summary

Warsaw Rehabilitation and Healthcare Center in Warsaw, NC is hiring an Occupational Therapist (OT) to join a growing team. The role focuses on comprehensive OT evaluations, treatment planning, documentation, and collaboration with the interdisciplinary team.

Requirements include an OT degree recognized by AOTA, current license, and SNF experience preferred. Flexible schedule and competitive benefits are offered.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred
• Current license/registration in the state (as applicable)

Responsibilities

• Provide a comprehensive OT evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop comprehensive treatment plans with goals, frequency and modalities.
• Ensure MD orders for evaluations, treatments, recertifications and discharges.
• Provide treatment using modalities and adjust as indicated while following precautions.
• Document daily participation and progress in progress notes.
• Update treatment plan monthly on recertification.
• Document discharge on discharge summary as indicated.
• Complete all required documentation and in-service notes.
• Document care plans and MDS information as needed.
