The Erie County Medical Center Corporation (ECMC) seeks a Pharmacist to provide patient-focused pharmacy services and collaborate with the patient care team to optimize medication use in a hospital setting.

Responsibilities include advising prescribers and staff, reducing costs, developing education programs, participating in rounds and research, and mentoring students. Licensure in NY and completion of ASHP-accredited residency preferred; one year clinical experience required.

Qualifications

• Graduation from a regionally accredited or New York State registered school of pharmacy.
• One year of clinical pharmacy experience.
• Completion of one year of ASHP-accredited post-graduate pharmacy residency.

Responsabilités

• Assists in planning, organizing, implementing and evaluating clinical pharmacy programs.
• Advises prescribers, nurses and patients on drug contents, actions and interactions.
• Documents cost savings and patient care interventions.
• Tracks pharmaceutical utilization, costs and prescribing trends to identify improvements.
• Participates in patient care rounds to optimize pharmaceutical management.
• Mentors pharmacy students, interns and residents.
