Job summary

Centerpoint Medical Center in Independence, MO seeks a Clinical Pharmacy Specialist to provide comprehensive clinical services and support on the hospital team. The role emphasizes monitoring patient regimens, ensuring safe and effective pharmacotherapy, and contributing to formulary decisions.

Responsibilities include participation in rounds, drug utilization, and education of staff and students, with a focus on delivering high-quality patient care in a fast-paced hospital setting.

Qualifications

• Pharmacist degree from ACPE-accredited school; PharmD preferred.
• Completion of hospital PGY1 residency or equivalent.
• PGY2 specialty residency or fellowship preferred.
• Board certification in pharmacotherapy (BCPS) required and maintained.
• Other certifications (FCCP, FASHP) preferred.

Responsibilities

• Interpret orders and verify them into the pharmacy computer system.
• Accurately compound and dispense pharmaceuticals.
• Complies with facility policy and regulatory requirements.
• Monitor drug therapy regimens for contraindications and interactions.
• Provide clinical consultation and suggest cost-effective therapeutic alternatives.
• Sustain the hospital formulary and promote rational drug therapy choices.
• Attend and participate in daily multidisciplinary and bedside rounds.
• Support formulary decisions and drug utilization improvements.
• Provide accurate medication information to healthcare staff and patients.
• Participate in antimicrobial management and other clinical programs.
• Document clinical activities and interventions; mentor and educate staff.
• Support quality improvement, risk management and patient safety initiatives.
• Complete notification reports regarding medication errors and adverse drug reactions.
• Participate in training of new pharmacists, students and residents.

Education

B.S. or PharmD from ACPE-accredited School of Pharmacy

Doctor of Pharmacy from ACPE-accredited college

Hospital PGY1 residency or fellowship OR relevant hospital experience

PGY2 specialty residency or fellowship

Board certification in pharmacotherapy (BCPS)

Other certification (FCCP, FASHP)
