Job summary

Centerpoint Medical Center in Independence, Missouri is seeking a Clinical Pharmacist PRN to join our hospital team. You will provide comprehensive pharmaceutical services for inpatients and help promote safe medication therapy while contributing to leadership of the medication use system.

In this role you will interpret orders, compound, monitor regimens, advise on alternatives, and sustain the formulary with clinical support across services.

Qualifications

• Pharmacist degree from ACPE-accredited program; Doctor of Pharmacy preferred.
• Active pharmacist license in good standing with the state board.
• State-specific Medication Therapy Management certification.
• Minimum hospital clinical pharmacy experience: 1 year or 2 years in a hospital setting.
• Hospital PGY1 residency or fellowship completion preferred.
• BCPS certification preferred.

Responsibilities

• Interpret orders and transcribe into the pharmacy system.
• Accurately compound and dispense pharmaceuticals.
• Monitor drug therapy for contraindications, interactions, allergies, and dosing.
• Suggest cost-effective therapeutic alternatives to clinicians.
• Sustain the hospital formulary and promote rational drug therapy.
• Provide clinical services such as drug consults, anticoagulant monitoring, pharmacokinetics, IV to PO, renal dosing.
• Complete notification reports for medication errors and adverse drug reactions.
• Provide oversight of staff schedules.

Education

B.S. or PharmD from ACPE-accredited School of Pharmacy

Doctor of Pharmacy from an ACPE-accredited college of pharmacy (preferred)

Active pharmacist license in good standing

State Medications Therapy Management Certification

Hospital PGY1/Pharmacy General Practice Residency or fellowship (preferred)

Specialty BCPS certification (preferred)
