Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to perform telephonic patient consultations on specialty medications, reconcile medications, and screen for interactions. The role involves coordinating with prescribers, expediting orders, and maintaining high-quality patient care.

The position requires ACPE-accredited training, active pharmacist licensure, and a strong clinical background with experience in specialty pharmacy preferred.

Qualifications

• Pharmacist with ACPE-accredited degree and active license.
• Experience in specialty pharmacy or clinical consultations desirable.
• Ability to meet quality standards and regulatory requirements.

Responsibilities

• Conduct telephonic patient consultations for specialty medications.
• Document clinical interventions in the pharmacy system per policy.
• Review prescriptions for strength, directions, and diagnosis; contact providers for clarification.
• Coordinate with physicians to optimize therapy and expedite orders.
• Back up pharmacists and train junior staff as needed.
• Maintain patient-centered, ethical care and comply with laws and ISMP guidelines.
