Job summary

Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed‑door, outpatient pharmacy at Butterworth Hospital in Grand Rapids, MI. It dispenses Specialty and Mon-Specialty medications for complex disease states with a multidisciplinary approach.

The Clinical Pharmacist will ensure safe medication use, supervise staff, and maintain regulatory compliance as part of an expanding care model. On-site with potential minimal remote work during a transitional phase.

Qualifications

  • Bachelor's or PharmD from an accredited college
  • 2 years hospital pharmacy experience
  • Preferred ASHP-accredited residency in Pharmacy Practice
  • MI Pharmacist license within 90 days

Responsibilities

  • Provide pharmaceutical care according to professional and departmental standards and regulatory requirements.
  • Provide safe, effective pharmacotherapeutic recommendations through medication management programs.
  • Supervise Pharmacy Technicians and Interns in daily operations.
  • Ensure pharmacist-in-charge responsibilities and compliance with state/federal regulations.
  • Educate patients and healthcare team on medication therapy and cost-effective practices.
  • Operate with medication automation systems and interpret orders for safety and efficacy.
  • Perform drug utilization evaluations and optimize dosage regimens to improve outcomes.

About Corewell Health Grand Rapids Hospitals Retail Pharmacy Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed-door, outpatient pharmacy located at Butterworth Hospital in Grand Rapids, Michigan. This pharmacy operates under a “Specialty-Plus” Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis. Scope of Work
  • Provides pharmaceutical care according to professional and departmental standards and statutory requirements.
  • Provides safe, effective, rational, and cost-conscious pharmacotherapeutic recommendations through the department's medication management programs.
  • The Clinical Pharmacist assists in supervising the functional activities of Pharmacy Technicians and Interns.
  • The clinical pharmacist has state of Michigan pharmacist in charge responsibilities for this pharmacy location including compliance to all regulatory requirements.
  • Maintains accountability for ensuring the safety and efficacy of medication use.
  • Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations.
  • Works with multiple medication automation systems.
  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
  • Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
  • Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
  • Weekends would be as needed.
  • This position would be On‑Site for the most part but could have a very small component of Remote work once everything is in more of a sustainable phase.
Qualifications
  • Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy 2 years of relevant experience hospital pharmacy
  • Preferred American Society of Health‑System Pharmacists (ASHP) accredited residency in Pharmacy Practice
  • Preferred LIC‑Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
  • LIC‑Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
  • CRT‑Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required
  • Or CRT‑Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required
  • Or CRT‑Basic Life Support (BLS) - AHA American Heart Association 180 Days required
  • Ideal candidate will be a Pharmacist possessing PharmD Degree and/or PGY1 Residency and/or PGY2 in Retail / Community / Ambulatory Pharmacy
  • Ideal candidate will also possess strong understanding of retail or outpatient pharmacy operations, will proactively ensure compliance with requirements as well as critically evaluate internal processes for continuous improvement
  • As this role is expected to have significant growth in the next 6 months, ability to be flexible and to adapt to changing environment will be critical
