Job summary

Advocate Health seeks a pharmacist for API Specialty Pharmacy - Retail Pharmacy in Wisconsin. The role is full-time with 40 hours weekly, 8am-6pm Monday to Friday, with occasional Saturdays. After six months onsite, remote work becomes available as part of a hybrid schedule.

Required: license to practice in Wisconsin, board certification in area of practice or equivalent, and 3+ years of licensed pharmacist experience including residency. Education: PharmD or P.B.

Qualifications

• Doctorate or Bachelor of Pharmacy from ACPE-accredited school or NABP FPGEC certificate.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on medication therapy, adverse effects, adherence and administration.
• Designs and evaluates patient-specific regimens based on literature.
• Implements recommendations, documents processes per policy.
• Reviews medication orders and prescriptions.
• Participates in medical emergency management when needed.
• Supports medication safety and quality improvement activities.
• Collaborates to manage formulary adherence and MUSE processes.
• Assists in acquiring non-formulary or investigational medications.
• Prepares, dispenses, and stores medications per regulations.
• Serves as a preceptor for students and residents.
