Advocate Health’s API Specialty Pharmacy – Retail Pharmacy is seeking a licensed pharmacist to oversee patient care, medication therapy, and safety within a 40-hour week. Responsibilities include counseling, medication order reviews, and collaboration with the healthcare team.

Position requires Doctorate or Bachelor in Pharmacy, state licensure, and residency experience. After six months onsite, the role may transition to remote work, with benefits and professional development from Advocate

Formación

• Pharmacist licensed in practice state or eligible for reciprocity
• Board certification in practice area or regional ladder certification preferred
• Immunization and non-vaccine injectable certification as required by practice area
• Credentialing and privileging as needed

Responsabilidades

• Oversees pharmaceutical care including literature evaluation and designing therapy regimens
• Counsels patients and caregivers on medication use, side effects, adherence and administration
• Reviews medication orders and prescriptions for accuracy and compliance with policies
• Participates in medical emergencies when applicable
• Contributes to medication safety and quality improvement initiatives
• Assists formulary management and non-formulary/investigational medication processes
• Ensures compliance with all regulations and organizational policies
• Precepts and mentors pharmacy students, residents, and staff as assigned
