Job summary

Prisma Health is seeking a Clinical Pharmacist Specialist to collaborate with a multidisciplinary team to improve patient outcomes through comprehensive medication management and education. The role includes direct patient care, therapy care plan development, and participation in hospital committees.

The incumbent will have authority to review histories, orders, labs, and adjust therapy according to scope of practice, with weekend and holiday coverage as needed.

Qualifications

• Bachelor's degree from ACPE-approved college of Pharmacy; PharmD preferred.
• Two residency programs or 10 years of specialty pharmacy experience, or 1 residency with 4 years of specialty experience.
• Active pharmacist license or eligible within 4 months of hire; BCPS/BCACP certifications preferred in ambulatory settings.

Responsibilities

• Provide direct patient care and clinical services within the department.
• Participate in committees and education programs; promote pharmacy profession.
• Mentor students and residents; lead clinical initiatives and evidence-based practice.
