Prisma Health in Columbia, SC is seeking a Clinical Pharmacist Specialist to provide direct patient care and collaborate within the department to enhance clinical programs and operations. The role includes education, committee participation, and mentoring pharmacy students and residents, with weekend, evening, and holiday shifts typical.

As part of a multidisciplinary team, you will develop therapy plans, manage medications, monitor outcomes, and provide timely drug information.

Qualifications

• Pharmacist degree and licensure within four months of hire.
• Two residencies or ten years of specialty pharmacy experience; or one residency with four years.
• ACPE-accredited degree with license or pending reciprocity.

Responsabilités

• Provide medication order-entry, distribution, and patient care; ensure appropriate therapy across age groups.
• Collaborate with multidisciplinary team to improve care and outcomes.
• Mentor students, residents, and technicians; support education initiatives.
• Lead or participate in department and institutional committees and projects.
• Provide drug information and on-call services; document in patient records.
• Advance evidence-based pharmaceutical care and participate in quality improvement.
