Job summary

UW Health is seeking a per diem Clinical Pharmacist to support inpatient care at University Hospital in Madison, WI. This role offers 20% FTE with day/evening shifts and a focus on medication therapy management within an interdisciplinary team.

Requirements include a PharmD or BS in Pharmacy, Wisconsin licensure within 90 days, and BLS/ACLS within the stated timelines. Tuition benefits and flexible scheduling are available to eligible staff.

Qualifications

• Bachelor's Degree in Pharmacy or a PharmD degree required.
• ASHP Accredited Residency training program preferred.
• 1 year of clinical patient care and orders management experience required.
• 3 years of institutional experience or completion of an ASHP accredited residency training program preferred.
• Licensed to practice pharmacy in Wisconsin within 90 days required.
• BLS within 90 days required.
• ACLS within 180 days required.
• Certifications (e.g., BPS, NCS, ACOGPS, etc.) preferred.

Responsibilities

• Provide medication therapy management services as part of an interdisciplinary inpatient team.
• Precept UW–Madison School of Pharmacy students.
• Join a cohesive team of collaborative pharmacists and support safe, effective, and cost-efficient pharmacotherapy.
• Collaborate with other health professionals to optimize patient care.

Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
