UW Health in Madison, WI seeks a Clinical Pharmacist - Critical Care to join an interdisciplinary team in inpatient services. You will provide medication therapy management, participate in bedside emergencies, and support safe, effective, and cost-efficient pharmacotherapy.
The role includes rounds, therapy monitoring, order verification, patient education, and precepting residents and students. 100% FTE with evening shifts 12:30 PM–10:00 PM and 1 in 4 weekends/holidays; UW Health offers
100% FTE, evening shifts varying between 12:30 PM - 10:00 PM. 1 in 4 weekends and holiday rotation required. Hours may vary based on the operational needs of the department.

Provide medication therapy management services as part of the interdisciplinary team to patients on inpatient services to assure that drug therapy is safe, effective, efficient, and cost-effective.
Bachelor's Degree in Pharmacy or a PharmD degree Required
Completion of an ASHP Accredited Residency training program Preferred
1 year of clinical patient care and orders management experience Required
3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
