Job summary

LiveWell Homecare Agency is seeking a licensed pharmacist to provide patient-centered medication management in a fast-paced, clinically focused environment.

The role involves preparing and dispensing medications, monitoring and adjusting therapy, supervising pharmacy staff, and promoting safety and efficiency in medication use. Missouri licensure and RPh are required; Doctorate preferred with 2-5 years of experience.

Qualifications

• Bachelor’s Degree in Pharmacy required.
• 2+ years of hospital/clinical experience required.
• Licenses such as MO Medication Therapy Services and RPh required.
• Doctorate in Pharmacy and 2-5 years of experience preferred.

Responsibilities

• Supervise pharmacy support staff.
• Participate in departmental meetings and committees.
• Serve as a medication therapy resource.
• Evaluate medication orders for appropriateness.
• Promote efficiency and safety within the medication process.
