Responsibilities

• Reviews and verifies medication orders with patient-specific parameters (diagnosis, weight, age, allergies, organ function, labs, interactions).
• Documents and communicates pertinent information with the healthcare team and pharmacy staff to ensure continuity of care.
• Provides drug information to physicians, nurses and other health care professionals.
• Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
• Promotes a culture of patient safety by submitting medication-related events and near misses.
• Completes daily pharmacy surveillance tool monitoring and follow-up with patient care initiatives.
• Works collaboratively with all pharmacists, pharmacy technicians and personnel; upholds respect and teamwork in the department.
