Beth Israel Deaconess Hospital – Needham is seeking a licensed pharmacist to join the expanding Beth Israel Lahey Health team. You will provide pharmaceutical care to patients, review medication orders, and ensure safe, effective therapy in accordance with state and federal regulations.

The role includes supervising students and staff, supporting formulary stewardship, and contributing to performance improvement activities.

Qualifications

• PGY-1 Residency or 3-5 years relevant hospital experience.
• Active Pharmacist license in Massachusetts required.
• BPS Certification, preferred.

Responsabilités

• Practices in accordance with state and federal laws, rules, and regulations for pharmacy.
• Maintains certifications and credentials in line with regulatory bodies.
• Reviews and verifies medication orders with patient-specific considerations.
• Clarifies inappropriate or unclear orders with the prescriber in a timely manner.
• Documents and communicates pertinent information with the healthcare team to ensure continuity of care.
• Provides drug information to physicians, nurses and other health care professionals.
• Supervises students, technicians, and residents within pharmacy practice.
• Promotes patient safety by reporting medication-related events and near misses.
• Participates in daily surveillance and performance improvement activities.
• Collaborates with pharmacists, technicians and staff to optimize care.

Formation

Licensed Pharmacist in Massachusetts

PGY-1 Residency or 3-5 years relevant hospital experience

BPS Certification, preferred
