Job summary

Advocate Health seeks a licensed pharmacist to oversee pharmaceutical care, evaluate literature, and design individualized therapy regimens. You will counsel patients and caregivers on proper medication use and adverse effects, while ensuring compliance and safety.

You will review orders, participate in emergencies, and contribute to formulary management and quality improvement. Mentorship of students and staff will be part of the role.

Qualifications

• Licensed pharmacist in state of practice or eligible for reciprocity
• Immunization and non‑vaccine injectable certification may be required
• ACPE‑accredited PharmD or NABP FPGEC certification required

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
• Counsels patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and administration
• Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on literature; implements and documents per policy
• Reviews and interprets medication orders and prescriptions accurately
• Participates in medical emergencies when applicable
• Supports medication safety and quality improvement activities, including reporting events
• Performs QA and performance improvement of the medication-use system
• Collaborates to manage formulary adherence and medication-use process; assists in acquiring non-formulary, compassionate use, and investigational meds
• Complies with policies, procedures, and regulations
• Prepares, dispenses, and oversees storage/handling of medications according to best practice and regulations
• Serves as a preceptor for pharmacy students, residents, and learners
• Mentors, orients, trains, and evaluates pharmacy teammates and students
• Directs/checks accuracy of pharmacy support team work to ensure safe workflows and compliance
• Demonstrates age-appropriate care across life span; adheres to department standards
