Job summary

Jefferson Health is seeking a Clinical Pharmacist to oversee the medication distribution system within their care area and to ensure each medication is indicated, effective, safe, and convenient for patients at Kennedy University Hospitals, Inc. in Stratford, NJ.

The position requires a Doctorate in Pharmacy or active pharmacist licensure with Pennsylvania registration, experience in pharmacokinetics, and the ability to monitor patient responses and document events.

Qualifications

• Doctorate Doctor of Pharmacy or RPH
• Knowledge of pharmacokinetic principles required for medication dosing
• Monitor patients for response to medications
• Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions
• Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee
• Provide care appropriate to patient age and background
• RP - Registered Pharmacist - State of Pennsylvania
• Preferred: 1 year PGY1 Pharmacy Residency
• Preferred: MS Word and Outlook skills
• Certification required based on primary work location

Responsibilities

• Interacts with co-workers, visitors, and other staff consistent with Jefferson values
• Assures the appropriateness of drug therapy and compliance with hospital policies
• Oversees the distribution of medications throughout TJUH
• Supports implementation of processes to assure the quality of Pharmacy services
• Trains staff as assigned (e.g., residents, interns, technicians, new pharmacists)
• Precepts student pharmacists as assigned (Preceptor Development & Evaluation Programpolicy)
