Job summary

UVA Community Health is seeking a Clinical Pharmacist to support personalized patient care across multiple facilities in Culpeper and surrounding areas. This role emphasizes medication safety, patient collaboration, and involvement in direct care within the UVA Health system.

The pharmacist will participate in rounds, perform assessments, and contribute to care plans, while maintaining current knowledge and following policy.

Qualifications

• 4 Year/Bachelor's Degree in Pharmacy required and FPGEC Certification.
• 1 year of experience required or PGY1 residency OR 2 years as a licensed pharmacist in health system pharmacy.
• Virginia pharmacist licensure within 30 days of start date; BLS certification required; Board of Pharmacy Specialties certification or equivalent within 2 years where applicable.
• Ability to drive between facilities/locations and work in a high performing team.

Responsibilities

• Provides optimal medication management and promotes medication safety.
• Collaborates with healthcare team to assess, plan, implement and evaluate an individualized plan of care.
• Participates in direct patient care activities and rounding where available.
• Maintains up-to-date knowledge required for services and documents appropriately.
