Job summary

The Rector & Visitors of the University of Virginia (UVA Health Community Health) seeks a Clinical Pharmacist to join the care team at UVA Community Health in Culpeper, VA. The pharmacist will support personalized patient relationships, collaborate with providers, and manage care plans per current policies.

Responsibilities include medication management, clinical rounding where available, and ensuring safety, dosage appropriateness, and drug interactions.

Qualifications

• 4 Year/Bachelor’s Degree in Pharmacy required; FPGEC Certification.
• 1 year of experience required or PGY1 residency/fellowship; or 2+ years as a registered pharmacist in health system pharmacy.
• Current Virginia pharmacist licensure within 30 days of start; BLS certification required; board certifications as applicable.

Responsibilities

• Provide optimal medication management and safety across patient care teams.
• Perform patient assessments and participate in direct patient care activities and rounding.
• Monitor therapy for appropriateness, dosage, interactions, and adverse drug reactions; document accordingly.
• Precept residents and students; mentor pharmacy team and contribute to education.
