Pharmacists ensure safe, effective, timely, patient-centered and evidence-based care. The role requires interpreting and evaluating medication orders within the interdisciplinary team and mentoring pharmacy

Qualifications

• Bachelor's degree in Pharmacy required.
• Doctorate in Pharmacy preferred.
• PGY-1 Residency or 1–3 years hospital experience required.

Responsibilities

• Interprets and verifies medication orders considering patient parameters such as diagnosis, weight, age and labs.
• Documents and communicates with the healthcare team to ensure continuity of care.
• Supervises pharmacy technicians, interns and new pharmacists.
• Promotes patient safety by reporting medication events and near misses.
• Assists in developing and revising departmental clinical guidelines.
