Job summary

UMass Memorial Health is seeking a Pharmacist to join our Memorial Prescription Center team in Central Massachusetts. This position is non-exempt with a 40-hour work week, and a rotating early to late shift including Saturdays.

The role emphasizes drug therapy management, patient counseling and collaboration with a multidisciplinary team to ensure optimal outcomes. Qualified candidates hold a Pharm.D or BSPharm, MA license, and at least 3 years ambulatory experience.

Qualifications

• Doctor of Pharmacy (Pharm.D) degree or Bachelor of Science in Pharmacy (BSPharm) degree.
• Licensed in the Commonwealth of Massachusetts.
• 3 years ambulatory experience.
• Essential skills to implement motivational interviewing to assist patients in active participation in health care.

Responsibilities

• Participates in drug therapy management and documents the care in patients' medical records.
• Prepares medications by reviewing and interpreting physician orders and detecting incompatibilities.
• Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
• Confers with physicians and other practitioners to optimize drug therapy and advise on interventions.
• Assists patients in gaining access to medications and resolves payer issues when applicable.
• Interprets clinical decision support and drug utilization reviews and responds accordingly.
• Provides health promotion and immunizations where permitted.
• Supervises technicians as needed.
• Complies with state/federal drug laws and regulations.
• Counsels patients on drug therapies and reports adverse events.
