Advocate Health is seeking a pharmacist to oversee pharmaceutical care, design therapy regimens, and educate patients and caregivers on medication use and safety. The role includes reviewing orders, participating in safety and quality initiatives, and collaborating with the care team on formulary adherence.

The position also involves precepting students and staff, mentoring teammates, and ensuring compliance with all relevant policies and regulations across the medication-use system.

Qualifications

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school
• Licensed in state of practice or eligible for reciprocity
• Immunization/injectable certification may be required

Responsabilités

• Oversees pharmaceutical care and designs medication therapy regimens from biomedical literature
• Counsels patients and caregivers on medication therapy, adverse effects, and compliance
• Reviews and interprets medication orders and prescriptions accurately
• Maintains medication safety, quality improvement, and reporting of events
• Collaborates with healthcare team on formulary adherence and meds process
• Precepts pharmacy students, residents, and learners; mentors staff
• Ensures compliance with policies, procedures, and regulations
