Klamath Tribal Health & Family Services seeks a Clinical Staff Pharmacist I to collaborate with primary care providers and staff, ensuring safe drug therapy, dispensing medications, and supporting MTMs and patient education across community sites within the Klamath Tribes’ Service Unit.

You will use electronic health records, participate in quality improvement, and coordinate with prescribers. Requires active pharmacist license, PGY-1 or 2 years experience, and flexibility for varied hours.

Formación

• Active Pharmacist license (Oregon license within 90 days if out of state).
• PGY-1 residency or 2 years of applicable experience required.
• Knowledge of Federal/state laws and regulatory requirements.

Responsabilidades

• Collaborate with pharmacy and primary care teams to provide high quality patient care.
• Ensure accurate preparation and dispensing of pharmaceuticals per professional standards.
• Perform medication therapy management and counseling.
• Manage inventory and ensure timely prescription delivery.
• Use EHR to document patient care and coordinate with providers.
