Job summary

SSM Health seeks a Clinical Pharmacist to deliver patient-centered medication management within an integrated Catholic, not-for-profit health system. You will verify and optimize medication therapies, provide clinical consults, and collaborate with interdisciplinary teams to ensure safe, evidence-based care.

Responsibilities include MTM, order verification, therapeutic drug monitoring, patient counseling, and precepting residents.

Qualifications

• Active Pharmacist license (RPh/PharmD) in the relevant state.
• Completion of ASHP-accredited PGY1 pharmacy residency preferred.
• Board certification in pharmacotherapy or relevant specialty preferred (e.g., BCPS).
• BLS and ACLS certifications as required by department.
• Eligibility to precept pharmacy students/residents preferred.

Responsibilities

• Provide clinical pharmacy services including MTM and optimization of therapies.
• Review and verify medication orders for safety, accuracy, and appropriateness.
• Collaborate with physicians, nurses, and care teams on evidence-based medication plans.
• Participate in patient counseling, education, and discharge medication reconciliation.
• Monitor patient response to drug therapy and adjust regimens per protocols.
• Support antimicrobial stewardship and formulary management initiatives.
• Ensure compliance with regulatory, accreditation, and safety standards.
• Precept and mentor pharmacy students or residents as assigned.
• Clinical pharmacotherapy and MTM responsibilities.
• Order verification and medication safety tasks.
• Therapeutic drug monitoring (e.g., vancomycin, anticoagulants).
• Drug information and evidence-based practice.
• EHR and CPOE proficiency.
• Interdisciplinary team collaboration and communication.
