PharMerica in Sacramento, CA is hiring a full-time Pharmacist for a non-retail, long-term care setting. You’ll evaluate orders, provide clinical recommendations, and maintain patient profiles in a collaborative team environment.

The role supports ongoing training, career growth, and advancement within a growing company. Overnight shifts operate on a 7 on 7 off schedule, with comprehensive benefits and a focus on high-quality care.

Qualifications

• Bachelor’s Degree in Pharmacy required; Pharm D preferred.
• One year experience in long term care desired.
• Active pharmacist license in the state of practice long term care knowledge is a plus.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes.
• Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
• Ensure lab monitoring for high-risk meds is ordered and communicated to the team.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical support.
• Document communications and interventions in the patient’s medical record.
• Enforce policies for safe handling, dispensing, and management of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication-related errors to the Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
