Job summary

PharMerica, an affiliate of BrightSpring Health Services, seeks a Pharmacist in Horsham, PA for a full-time, closed-door pharmacy role serving long-term care and senior living facilities. Schedule is Monday–Friday 3:00pm–11:00pm with every other weekend; shift differential of $2.50/hr.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dose, interactions and administration route.
• Provide clinical recommendations to physicians and facility staff.
• Maintain patient/resident profiles including medical history and current medications.
• Ensure monitoring for high‑risk medications is ordered and communicated.
• Collaborate with physicians, nurses, and healthcare professionals for support.
• Document communications and interventions in the medical record.
• Ensure compliance with regulatory requirements for pharmacy practice and medication management.
• Report medication-related errors to the Pharmacy Manager.
• Educate facility staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
