Job summary

PharMerica in Washington, NC is seeking a full-time Pharmacist for our non-retail long-term care and senior living pharmacy. You will evaluate orders for appropriateness, provide clinical recommendations, maintain patient profiles, monitor high‑risk medications, and collaborate with medical staff.

Qualifications

• Bachelor's Degree in Pharmacy or PharmD preferred.
• 1 year experience in long-term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS/DEA/FDA guidelines and medication safety.
• Proficient in EMR and pharmacy information systems.
• Strong clinical judgment and communication skills.

Responsibilities

• Evaluate physician orders for appropriateness, dosage, interactions, and route.
• Provide clinical recommendations to physicians and facility staff.
• Maintain up-to-date patient profiles including medical history and meds.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with healthcare professionals to support therapy changes.
• Document interventions in the medical record and with families.
• Enforce policies for safe procurement, storage, and disposal of meds.
• Ensure compliance with state and federal pharmacy regulations.
