Job summary

PharMerica is hiring a full-time Pharmacist in Washington, NC to join our non-retail, closed-door pharmacy team serving long-term care and senior living clients. You will evaluate orders, optimize medications, maintain resident profiles, and collaborate with physicians and nurses to ensure high-quality care.

Qualifications

• Bachelor’s degree in Pharmacy required or PharmD preferred.
• One year of experience in long-term care is desirable.
• Current and valid pharmacist license in the state of practice.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and facility staff on medication therapy changes.
• Maintain patient/resident profiles with medical history, labs, and current meds.
• Ensure monitoring labs for high-risk meds are ordered and communicated to the team.
• Collaborate with physicians, nurses, and other healthcare professionals for support.
• Document all clinical interventions in the patient’s medical record.
• Enforce policies for safe procurement, storage, administration, and disposal of meds.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to the Pharmacy Manager.
• Educate facility staff on safe medication handling procedures.
