PharMerica in Columbia, MD is seeking a full-time Pharmacist to join a non-retail, long-term care and senior living focused team. You will evaluate orders, provide clinical recommendations, and maintain patient profiles to ensure safe and effective medication therapy.

The role involves collaboration with physicians and staff, regulatory compliance, and ongoing training, with weekday shifts 3pm-11:30pm and alternating weekends. Competitive benefits and growth opportunities are offered.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• Current and valid pharmacist license in the practicing state.
• Knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA).
• Experience with medication safety practices and regulatory compliance.
• Proficiency with electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, and interactions.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain patient/resident profiles including medical history, labs, and meds.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and other professionals for care.
• Document communications and interventions in the medical record.
• Enforce safe handling, dispensing, and oversight of medications.
