Job summary

PharMerica in Largo, FL is hiring a full-time Pharmacist for a non-retail, long-term care setting serving senior living clients.

Qualifications

• Requires Pharmacist license in Florida and degree in Pharmacy.
• Experience in long-term care is preferred.
• Knowledge of CMS/DEA/FDA guidelines.
• Strong clinical judgement and communication.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, and interactions.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain patient profiles with medical history, labs, and medications.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and other professionals for patient care.
• Document interventions in patient records, including family communications.
• Enforce safe handling, dispensing, and medication management policies.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors to the Pharmacy Manager.
• Educate clinical teams on safe medication handling.
• Perform other pharmacy tasks as assigned.
